A. Educational Eligibility

C. Original and Clear Copy of PSA Birth certificate and Original and Clear Copy of PSA Marriage Certificate (for married female applicants)

Submitting an original and clear copy of the PSA (Philippine Statistics Authority) birth certificate is a crucial requirement for applicants in the licensure examination.
